易瑞沙的耐药机制是怎么样的 与其他抗肿瘤药物相似，易瑞沙是治疗乳腺癌的佼佼者，但治疗中也存在原发性耐药与继发性耐药的情况，耐药机制主要表现为一下两个方面。 
临床大量资料表明：根据肺癌患者的实际情况选用易瑞沙（吉非替尼）治疗后，仍有部分肺鳞癌和其它非小细胞肺癌的患者效果比较明显，且具有良好的耐受性。但所有患者在使用一段时间后均会出现不同程度的耐药性。 

蛋白酪氨酸家族是人类肿瘤中最常发生突变的基因家族。约10%的肺腺癌患者中发现了编码HER2蛋白酪氨酸激酶区的HER2基因突变，HER2突变型较野生型更具侵袭性和致瘤性，突变型对HER2靶向治疗敏感，但对EGFR酪氨酸激酶抑制剂吉非替尼不敏感。ErbB家族基因突变与靶向治疗及预后有关。胞内的信号通路互相联系，形成一个复杂的网络体系，易瑞沙虽然能够抑制HER2同源或异源二聚体的形成从而阻断下游信号通路，但肿瘤细胞可通过膜表面其它受体旁路激活HER2的下游信号通路，导致其耐药现象发生。 

解决易瑞沙耐药的对策随着该药耐药机制的研究深入，人们也研制了许多针对性强的干预药物，该药与紫杉醇联用均可显著增强抗ErbB2 高表达肿瘤的作用，有望发展成为一种独具特色的临床候选药物，并可提高疗效。
