[Indications] It is a hepatitis C virus nucleotide analog B polymerase inhibitor suitable for the treatment of chronic Hepatitis C (Hepatitis C) infection as a component of a combination antiviral treatment regimen.
Efficacy has been established in subjects with genotype 3 or genotype 3 infection, including those with hepatocellular carcinoma meeting Milan criteria (awaiting for liver transplantation) and those with genotype 3 or -co-infection. The following points should be considered when initiating treatment:
Monotherapy is not recommended for treatment.
The treatment regimen and timing depend on both the virus genotype and the patient population.
Changes in treatment response based on baseline host and viral factors.
[Usage and Dosage] The recommended dosage is tablets, taken orally, once a day with or without food. In adults, treatment should be combined with ribavirin or with pegylated interferon and ribavirin. Co-administration with ribavirin may be considered as a treatment option in patients with genotyped infections who are ineligible for interferon-based regimens. Treatment decisions should be guided by an assessment of potential benefits and risks for the individual patient.
[Contraindications] When used in combination with ribavirin or peginterferon-ribavirin, the contraindications that apply to these drugs also apply to the combination therapy. See the prescribing information for peginterferon and ribavirin for a list of contraindications.
Women who are or may become pregnant and men whose female partners are pregnant are contraindicated in concomitant treatment with ribavirin or peginterferon-ribavirin because of concomitant birth defects and fetal death with ribavirin.
[Precautions] Ribavirin may cause birth defects and/or death of exposed fetuses and animal studies have shown that interferon has abortive effects [see Contraindications]. Extreme care must be taken to avoid pregnancy in female patients and in the female partners of male patients.
Ribavirin therapy should not be started unless a negative pregnancy test is reported immediately before starting treatment.
When used in combination with ribavirin or peginterferon ribavirin, women of childbearing potential and their male partners must use two forms of effective contraception during and for at least 1 month after treatment has ended . Routine monthly pregnancy tests must be taken during this time. There are no data on the effectiveness of systemic hormonal contraceptives in women, therefore two non-hormonal methods of contraception should be used during treatment with and concurrent ribavirin.
